Nursing Services


Responsible for the hands on delivery of care and assistance with activities of daily living for all residents. Provides care in a manner that meets or exceeds Western States Senior Communities expectations.


Reports to: Charge Nurses, Director of Nursing

Supervises: None

Interfaces with: Residents, Charge Nurses, Families, Visitors, & Associates

  • Answers call lights to determine resident needs.
  • Assists residents with grooming, oral hygiene, bathing, hair and incontinence care.
  • Obtains food trays and assists residents with feeding.
  • Assists residents with range of motion exercises, and movement to wheelchair or activity areas.
  • Assists residents with turning and positioning in bed.
  • Assists residents with ambulation for short distances in the facility.
  • Takes and records temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.
  • Performs other tasks as assigned.
  • Conducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
  • Documentation on flow sheets as directed.
  • Attends interdisciplinary meetings as needed to provide input into selected resident's plan of care.
  • Documents restorative services provided following facility policies and procedures.
  • Reports to charge nurse or designee on progress/lack of progress in achieving restorative goals.
  • Maintains the dignity and privacy of all residents.
  • Protect all resident rights.
  • Assist residents with personal needs as requested, while helping to maintain the resident's independence and well being.
  • Maintain and protect the confidentiality of resident information at all times.
  • Meet or exceed Western Senior Communities standards of appearance; comply with Western States Senior Communities sanitation, hygiene and health standards of community personnel.


Completion of a State of Utah approved Certified Nursing Assistants course.

Must possess a current Nursing Assistants certificate or be able to obtain one within four months of employment.

Must possess a current Food Handlers permit.

Ability to effectively communicate with residents, families, supervisor, employees.

Willingness to work with the elderly.
